We began Life Options for Seniors to provide a more personal and in-depth connection with families faced with making life-changing decisions. Having worked in hospitals, Skilled Nursing Facilities, Home Health, and In-home, we have the medical background, skills, and experience required to provide you with appropriate care options.

Our mission

To guide and educate you during a stressful time and help you make an informed decision. We do the footwork for you. We visit each home to ensure they meet criteria for safety, collect specific information as to what type of care each home can accommodate, type of resident they prefer, if they allow pets. We also check with licensing to ensure that the facility does not have any prior violations.
How we work
Once a Referral has been made to Life Options for Seniors

We set up a face-to-face assessment usually in the comfort of your home or at a hospital setting, during which we assess your functional abilities, mental status, review medical diagnosis, review current medications, and assess financial situation.

Once we have determined your care needs we put together senior care options that can accommodate your medical needs and fit within your financial budget.

Implement plan of care. If the care plan requires transitioning into a senior care facility or bringing in-home care, we then contact each facility/agency to coordinate senior care services. We schedule a tour of each facility. We can accompany you to visit each facility and be there when an In-home care provider visits with you.

We follow up to ensure for a smooth transition and continue to monitor your care needs. We are always available.

Personalized Assistance – Our senior care consultant well meet with you face-to-face, contact your primary care physician to obtain necessary paperwork and follow up once you have transitioned.
Who we are

Sue Hoppe
Care Consultant

Sue Hoppe, Co-Owner of Life Options for Seniors, has been in the medical field for 35 years. Sue has worked with Case Management/ DC Planning, Physicians, Home Health, and Social Services, both at Kaiser Foundation Hospitals and Sutter Hospitals. Sue specialized in placements of patients into and out of skilled nursing facilities and acute rehab Facilities. She found it so rewarding to help patients and their families that she wanted to continue on a more personal level.
Her involvement with Life Options for Seniors have allowed her to do just that and led her to join up with Adopt An Elder Foundation, where she currently holds the position of Board Secretary. When Sue is not working, she enjoys spending time with her kids and grandkids and buying new toys for her dog Mickey.
Homa Rostami
Care Consultant

Homa is a Geriatric Care Manager and co-owner of Life Options for Seniors. Homa has worked in the senior living industry for over 15 years in a variety of roles. Homa has worked with Case Management and Care Coordination with Kaiser Foundation hospital, as an Admissions Director in a skilled nursing facility, and as a Marketing Director for an in-home care agency. Her exposure to different aspects of the medical field gives her a more in-depth knowledge of the senior care industry. Homa earned her bachelor’s degree from the University of Pacific and a degree in Gerontology from American River College. She is a certified administrator for residential care facilities for the elderly.
Her passion for helping seniors has led her to volunteer with Adopt an Elder foundation program at which she is past President and current Treasurer. She lives in Rocklin with her husband and son, Iziah. She enjoys reading, scrapping booking, and spending time with her family.


Pam Murphy
Care Consultant

Pam worked for the Area Agency on Aging for Santa Clara County as a Volunteer Coordinator for the Friendly Visiting Program for three years, later becoming Coordinator of the Registry for In-Home Support Services (IHSS) for the AAA of Santa Clara County for two years. When Pam moved to Roseville, she found herself again drawn to the senior care industry, working as a Marketing Director in an Assisted Living community for five years.
In 2010, she started working as a Senior Care Consultant- assisting families in need with placement and senior care services. Pam is current Board member of the Sunshine Committee in Sun City Roseville and a committee member for Senior of the Year Program. Pam’s desire for working with seniors led her to join up with Adopt An Elder in 2014, as the Adoptee Assessment Coordinator. In her spare time, Pam loves visiting her grandchildren, travel, and entertain.
Shelly is a certified administrator for residential care facilities for the elderly. She joined Life Options For Seniors in 2015 as a senior care consultant. She has always had a passion for working with older adults. Shelly is committed to helping older adults. Her desire to do so has led her to join the Walk to end Alzheimer’s committee in 2016, The Adopt an Elder Foundation Board in 2017 and The Elder Services Network advisory team in 2018. When Shelly is not working, she enjoys spending time with her husband and 2 kids. She also enjoys cooking, gardening, arts and crafts and traveling.
